The highest point of the ecliptic at your moment of birth.
The Midheaven begins the tenth house and is usually tied to career, public role, and how you're seen from outside.
A common misconception is that it sits exactly 90° from the Ascendant. That's only roughly true near the equator — the angle between them varies with latitude and can depart from 90° considerably further north or south.
